Bivalirudin Trifluoroacetate functions as a bivalent direct thrombin inhibitor by specifically and reversibly binding to both the catalytic site and the anion-binding exosite of thrombin. This dual binding mechanism effectively blocks thrombin's interaction with its substrates, inhibiting the conversion of fibrinogen to fibrin and preventing thrombus formation. Unlike indirect inhibitors like heparin, it acts independently of antithrombin III and can inhibit both circulating (free) and clot-bound thrombin, which is often inaccessible to other anticoagulants. This direct action results in a rapid and predictable dose-dependent anticoagulant effect, prolonging clotting times such as ACT and aPTT without causing heparin-induced thrombocytopenia.
