1.MECHANISM OF ACTION

Pentagastrin acts as a potent and selective agonist for the Cholecystokinin B/Gastrin receptor (CCK₂R). Upon binding to this G-protein coupled receptor, primarily located on gastric parietal cells and enterochromaffin-like (ECL) cells, it activates the Gq/11 signaling pathway. This triggers a cascade involving phospholipase C (PLC) activation, generation of inositol trisphosphate (IP₃) and diacylglycerol (DAG), leading to intracellular calcium mobilization and protein kinase C (PKC) activation. 

2.PRODUCT INDICATIONS

• This product is indicated for use in gastric function tests to diagnose conditions such as achlorhydria (in pernicious anemia or atrophic gastritis), Zollinger-Ellison syndrome, and post-surgical gastric acid assessment. It is also employed as a provocative test for medullary thyroid carcinoma (MTC) screening by stimulating calcitonin release. Researchers and clinical laboratories use it to evaluate the efficacy of antisecretory drugs (like PPIs and H2 blockers) and to study GI motility and pancreatic secretion in preclinical models.



3.CLINICAL EFFICACY OF PRODUCT

Diagnostic Accuracy: It is a reliable and standardized agent for evaluating maximal gastric acid output (MAO) and peak acid output (PAO). A subnormal response can indicate parietal cell loss (e.g., in atrophic gastritis), while an exaggerated response may suggest hypergastrinemia or gastrinoma.
Safety Profile: While generally safe, transient side effects (abdominal cramps, nausea, flushing, dizziness) are common. Serious adverse reactions are rare. Its safety profile is considered favorable compared to older tests like the histamine stimulation test.
Standard of Care: For decades, the pentagastrin stimulation test has been a gold-standard diagnostic tool in gastroenterology, though its routine use has declined with the advent of non-invasive Helicobacter pyloritesting and advanced imaging. It remains a valuable tool in specific diagnostic dilemmas.
